When you submit our appointment request form we collect your name, phone number, email address (optional), the reason for your visit, your preferred time, and any message you choose to include.

Like most websites, our hosting provider may automatically record technical information such as your IP address, browser type, and the pages you visit, for security and performance purposes.

We use the information you send us only to respond to your request, schedule your appointment, and communicate with you about your care. We do not sell, rent, or trade your personal information, and we do not use it for marketing without your consent.

Form submissions are delivered to our office email by a third-party email delivery service and are handled by our front desk team.

Please do not include detailed health information in the website form. Your dental and medical history is collected in person at the office, where it is stored securely as part of your patient record and used only for your care, billing, and as required by law.

This website is hosted on Vercel and uses Google Maps to display our location. These providers may collect technical data under their own privacy policies. We do not use advertising trackers on this site.

You may ask to see the personal information we hold about you, request corrections, or withdraw consent to future communications at any time by contacting our office. We will respond within a reasonable time and in accordance with applicable law.
